You will be asked to arrive 15 min before your scheduled treatment hour to fill out intake forms and enter your information in the computer system. Refer to Patient Forms section to download, print and complete these important forms ahead of time. This will expedite the registration process and allow your evaluation to start on time.
You and your physical therapist will then discuss your problem and history in more detail. The therapist will then evaluate your problem including assessing movement, performing tests, and taking measurements.
Treatment will begin during this first visit. This may provide you with some relief from your symptoms, and it also provides your therapist with a more hands-on assessment of your problem. Because your therapist will be assessing movement and performing treatment, it is important you wear clothing which will allow them to access the region for evaluation treatment.
You will be given a folder at the end of your first appointment. Your exercises, given as home work, will be kept in this folder. You may be given one or two exercises on your first visit, but don’t worry, we will avoid giving you exercises that are too difficult for you to perform. Try to bring your exercise folder with you every visit as we will continue to add, change, and update your home program as your status progresses. This folder also contains useful information on what you may experience after treatment and information on your insurance coverage.
You and your therapist will also decide on a treatment plan, establishing the number of sessions per week you will need. Our administrative staff will then work with you on scheduling follow-up appointments with your assigned team of therapists.

What do I wear to my treatment session?
For every area we address, we need to assess and treat the entire region. This is easier when the region is readily accessible. In addition, part of the session will involve exercises and therefore comfortable, loose fitting clothing is important. A tank top or loose fitting t-shirt works best for treating neck, shoulder, elbow, wrist and hand as well as mid-back. T-shirt and gym shorts for low back, hip, knee, foot and ankle are the most optimal. However, you are welcome to discuss other options with your therapist if you are uncomfortable wearing any of these items. We also have shorts and T-shirts available in the clinic if you forget to bring them to a session.
Are you a BCBS provider?
Yes we are members of the BCBS preferred provider network.
Can I pay by credit card?
You can come into either location (Huntsville or Madison) to make a payment by credit card. However, our business practice does not allow us to accept payments over the phone using credit cards.

Do you bill insurance companies for treatment due to a MVA?
Yes, we do bill insurance companies for treatment due to a Motor Vehicle Accident. We do need the claim information, insurance company name, and a point of contact to be able to set up billing for this situation.
Do you bill Workers Compensation?
Yes, we do. We do need the claim information, insurance company name and a point of contact to be able to set up billing in this situation. Each WC situation has slightly different requirements so we call to verify the requirement.

How do I handle payments?
You may pay on your account daily, weekly, monthly or set up an interest-free payment plan. We try to be flexible and reasonable regarding payments.

What if I have to cancel my appointment for the day?
Our cancellation policy states that we need a 24 hour notice or we will charge $25.00 for the missed visit. However, you don’t always have a 24 hour notice from your ill family member, so we try to be flexible and give you a little leeway on the cancellation charge. By giving us 24 hr notice, it allows us time to schedule someone else in that time that also needs an appointment, so your consideration of our time and schedule is greatly appreciated.
What is a Co-pay?
It is the amount of money that you pay each time you visit a therapist. Some insurance plans have a set $ amount, others have a percentage. If you have a deductible, you will pay the deductible first and then you will be charged for the co-pays.
What is a Deductible?
Deductible is the amount of money that your insurance plan expects you to pay before they will pay any portion of your medical expenses. Sometimes the deductible amount is applied one time under your plan. At other times, it is applied for each type of medical treatment i.e. psychiatric, speech therapy, physical therapy, chiropractic, etc. The deductible is usually a yearly charge.
Will my insurance cover this physical therapy?
Most insurance company will pay for physical therapy but the number of visits, co-pay, and deductible varies per insurance coverage. Will you bill my insurance companies?
Yes, we will bill your primary and secondary insurances except Medicaid
